Medicare to Audit Visits Billed With Nail Debridement
Via a recent Medicare audit of dates of services on which an office visit and nail debridement (11720, 11721) were both billed, they found most provider’s records do not justify separate billing of the visit. In correspondence with this, Medicare recently announced that they will be performing prepayment reviews of visits billed with a 25 modifier in conjunction with a nail debridement. It is likely that they will request medical notes prior to pay- ing such charges.

To fully comply with Medicare’s guidelines for separate reimbursement, a visit should be performed for an un- related problem to the nail debridement. When billing this combination, please make sure your medical notes clearly identify the separate problems being treated, and mark your diagnosis codes accordingly.
