New Missing Information Lists
Over the next few weeks, HCA will transfer your missing information list to a new format. On the new form, all items will be listed with a “Date of Request.” If any items are on the list for more than 3 months from this date, we will remove them from the list and our system. This change is necessary to streamline our account follow up procedures and maintain the most accurate missing information list.
Please click the link below to view a sample of the new list format. As always, when returning missing informa- tion, please use the list as a cover page so we are able to remove the corresponding items from the current list.
